Almost looks home made to me. I have never seen one of those. Not sure if you know, but the 400 emblem on the trunk lid is in the wrong location too.

Since you seem to know pretty well about these cars, what are the rectangular holes on the rear and both sides of the rear bumper ?

Slots for the bumper jack. Front bumper has them too. Emblem should be lower, along the bottom edge of the lid.

The trunk lid emblem on my car is also mounted too high. I guess the trunk lid has been changed some time.
