Here is some information that I obtained that was not decoded by the PHS service.
I found out my dealer by doing a title search and was able to decode my own zone. The first sate registered had a microfilm of the manufacturers statement of origin showing car transferred from Pontiac to the dealer also had invoice number. It provided the name next to the zone codes. back of form had assignment to owner
I sent my information to Triple A enterprises in 1996 ( 317 875 7636 of Indianapolis) to make a repro widow sticker. I asked them what all the boxes meant on the billing history that weren't decoded by PHS. They answered some of them for me (as much as they had knowledge of). I highly recommend them if they are still around. A hand written note by Bill Patrick/(hard to read last name) indicated a few options that were not decoded from the billing history by PHS. He said codes 55 4 and 77 X were my wheel covers. (they are deluxe wheel discs, std with custom trim). I think he meant 77x was a the turbo hydramatic 400 transmission because that is what the window sticker came back with for that option. There was no spot on the order sheet for Pontiac provided PHS to highlight these. He said he wasn't positive but thought 74 F was the axle ratio (3.08, standard ratio, not optional). Mine is a safe-t-track. I suspect these guys have the zone codes all figured out.
A document in the package from PHS warns not trying to decode all the options from the billing history as several codes were used to describe things such as "winter antifreeze" , and are not listed on the order form.
