Does anyone have a 69 order form with the left colum (titled UPC) legible? If so, can I get a copy of it? This is the same form that PHS sends with the package as a decoder to the invoice. Does anyone know how many different versions of the 69 order form there were during 69? There was the initial one and the 5/13/69 form that is sent out by PHS. Any others?
Also, does anyone have a copy of the 67 order form handy?
